The GUIDE Model

Guiding an Improved Dementia Experience

The GUIDE Model provides comprehensive, coordinated dementia care. It improves quality of life for people with dementia by enabling them to remain in their homes and communities and reducing strain on their unpaid caregivers. The GUIDE program provides care coordination and management, caregiver education and support, and respite services. GUIDE is available at no out-of-pocket cost to patients with traditional Medicare.

The GUIDE program can provide you and your loved one with…

  • A member of our team will visit the patient and caregiver at the home or virtually to assess and determine individual needs.

  • A Care Navigator will develop and implement a person-centered care plan led by the patient and the caregiver.

  • 24/7 access to a help line to assist with needs.

  • The Care Navigator will connect routinely via phone to provide ongoing support and monitoring and make changes to the care plan as needed.

  • A Care Navigator will coordinate medical and dementia care with the primary care provider, assist with medication management, and provide support after hospital and ER stays.

  • LifeCare Solutions will refer and connect the beneficiary and Caregiver to community-based services and supports that address the common needs of persons with dementia and their Caregivers.

  • The Care Navigator will work with the patient’s PCP and other health care providers to monitor medication adherence, assist with pharmacy packaging solutions, and monitor medication changes.

  • If a caregiver is identified, the care navigator will provide caregiver skills training including:

    • Dementia diagnosis information

    • Support group services

    • One-on-one support calls via 24/7 hotline

  • LifeCare Solutions will refer to contracted providers of in-home respite services, adult day centers and facilities. A beneficiary with an identified un-paid caregiver may be eligible to receive $2,500 per year for respite.

Eligible

  • Traditional Medicare beneficiaries (Part A & Part B) with a confirmed dementia diagnosis (including Alzheimer’s, LBD, FTD, vascular dementia, etc.) or a suspected dementia diagnosis

  • Living at home or in the community (Independent Living, Assisted Living & Memory Care)

Not Eligible

  • Medicare Advantage enrollees

  • Living in a nursing home

  • Participating in Hospice or a PACE organization

Approximately 10% of Medicare beneficiaries have dementia and may be eligible for GUIDE. This program offers unique care and support to those living with dementia. It aims to reduce caregiver strain, financial burden, and offer support to those in underserved areas.

Frequently Asked Questions

  • No. The GUIDE program is provided at no cost to the beneficiary. There are no out-of-pocket expenses for participating in the program or receiving approved services.

  • No. Participating in the GUIDE program does not impact your ability to use other benefits. However, you cannot be enrolled in GUIDE while receiving hospice services or while living in a long-term nursing home.

  • No. Anyone can refer, even yourself! Call us or email us today to inquire!

  • The $2,500 can be used to pay for respite services provided by Medicare approved GUIDE respite providers. These services are designed to give caregivers temporary relief while ensuring your loved one continues to receive quality care.

  • No. Respite services must be provided by approved GUIDE respite providers. Private caregivers or companies that are not approved are not eligible for reimbursement under this benefit.

  • GUIDE is only approved for Traditional Medicare. If you have a Medicare Advantage plan or if your primary insurance is a commercial or federal plan, you do not qualify. You can have a supplemental plan along with Traditional Medicare, or Tricare for Life with Traditional Medicare.

  • Medicare Advantage is a type of Medicare plan offered by private insurance companies. If you have a plan that is not Original (Traditional) Medicare, or if your plan has a name like HMO, PPO, or Part C, you likely have Medicare Advantage. You can also check your Medicare card—Original Medicare cards say “Medicare” and include only your name and Medicare number, while Medicare Advantage cards are issued by private insurance companies. Call us and we are more than happy to check your insurance coverage!

  • Yes. You can be dually enrolled in Medicare and Medicaid and still participate in the GUIDE program, as long as your Medicare coverage is Traditional Medicare.
