How does it work?

  • Visits are a minimum of 1.5 hours and are set on a routine basis. A typical schedule looks like one to two visits per week, though we are happy to adjust the schedule based on your family’s needs and our team’s availability.

  • While our Bridge program provides oversight and companionship, our Bridge team does not preform personal care tasks. Our team of professionals focus on companionship, socialization and oversight.

MEET JOHN
“John was isolated, living at home alone, had a stroke and needed memory care. His POA agreed to try our BRIDGE Program and I began meeting with him. The “small joys” of seeing a somewhat familiar face when you’re in an assisted living facility is like steadily receiving a fresh stash of dove chocolate for your room when you can't get out and buy them yourself and having activities to look forward to. Seeing people each week can make a world of a difference in someone's life. We are planning a “show-case” of John’s music, art collection and many contributions he made to his field, to be held in a few months - he is so excited and grateful for it. The thoughtful engagement is helping John feel remembered, appreciated and greatly valued. It is also giving his POA, in another state, peace of mind to know that John is not only being looked after, but also included and honored, with his quality of life made a priority instead of an afterthought. He has expressed appreciation repeatedly. We have also made a point to bring him special mementos from his house such as his piano sheet music, his commissioned artwork and beautiful China collection for him to be reminded of the things he loves.”
